We work with individuals, families and hospital teams to plan a smooth transition, reduce the worry around discharge and help people find their feet again.

    We carry out a needs and risk assessment and co-produce a care plan around your recovery goals.

  3. Home on day one

    Your carer is ready to support you as you arrive home, helping you settle in safely.

  4. Recover and review

    We review your plan as you regain confidence, reducing or adjusting support as you need.

Our team can usually have support up and running within 24 to 48 hours of your consultation and needs assessment.

Can a hospital or social worker make the referral?

Yes. We welcome referrals from hospital discharge teams, social workers and health professionals, as well as from families and individuals.

Is the support short-term?

It can be. Many people need support for a few weeks while they recover, but care can continue longer if needed.
